From 01\/04\/2021, a new form of health insurance card will be applied nationwide according to Decision No. 1666 of Vietnam Social Security. Here are some frequently asked questions around applying the new card template.<\/span><\/p>\n

  1. When will a new model health insurance card be issued?<\/b><\/li>\n<\/ol>\n

    When the local social insurance agencies use up the old template health insurance cards, they will issue a new form of health insurance cards.<\/span><\/p>\n

    1. Who is the new form health insurance card issued to?<\/b><\/li>\n<\/ol>\n

      After the social insurance agencies of the provinces use up the old template health insurance cards, a new form of health insurance cards shall be issued for the following cases:<\/span><\/p>\n

      – New participants in health insurance.<\/span><\/p>\n

      – The participant lost his\/her health insurance card.<\/span><\/p>\n

      – The participant has torn, damaged or changed the information on the health insurance card.<\/span><\/p>\n

      * Note: During the transition from the old model health insurance card to the new one, those who are using the old model health insurance card with expiry date can continue to use it for medical examination and treatment covered by health insurance and extend the validity of the health insurance card on the management database, do not print or change the health insurance card (except for the cases mentioned above).<\/span><\/p>\n

      1. Where to receive an application for new\/re-issue of health insurance card
        \n<\/b><\/li>\n<\/ol>\n

        a) At the social insurance agency in the following cases:<\/span><\/p>\n

        – Persons whose health insurance premiums are paid by the social insurance organization.<\/span><\/p>\n

        – Persons receiving partial support from the state budget for health insurance premiums.<\/span><\/p>\n

        – Participants in health insurance according to households.<\/span><\/p>\n

        – Persons receiving unemployment benefits that need to change their health insurance cards.<\/span><\/p>\n

        – People who have donated body organs.<\/span><\/p>\n

        b) At the agent collecting social insurance and health insurance in the following cases:<\/span><\/p>\n

        – Persons receiving partial support from the state budget for health insurance premiums.<\/span><\/p>\n

        – Participants in health insurance according to households.<\/span><\/p>\n

        c) At the Commune People’s Committee in the following cases:<\/span><\/p>\n

        – People whose health insurance premiums are paid by the state budget.<\/span><\/p>\n

        – Persons whose health insurance premiums are paid by the social insurance organization.<\/span><\/p>\n

        d) At the enterprise: The participants of health insurance pay at the enterprise and submit the application to the employer.<\/span><\/p>\n

        e) School: Students, students pay health insurance according to the school, submit the application to the school.<\/span><\/p>\n
